School Physical Health Examinations are available to children 3 – 18 years of age attending childcare, pre-kindergarten, public, and/or private schools.
Please note: Sports Physicals are not provided
What is included in the School Physical Health Examination?
- Complete Physical Health Examination
- Immunizations that are required for admittance into childcare, pre-kindergarten, public, and/or private schools
- Vision screening
- Height and Weight
- Lead screening at the Broward County Health Department:
- If your child is 1 – 6 years of age, and
- Uninsured
- Tuberculin (PPD) skin test at the Broward County Health Department:
- If your child has been exposed to adults at high risk for Tuberculosis (TB) disease, and/or
- Recent immigrant from a country with high rates of TB
What is the cost of the School Physical Health Examination?
The cost for the School Health Physical Examination is $36.00 payable by cash or credit card only.
Are there other costs that I will need to pay?
There may be additional charges if additional testing is recommended by the healthcare provider.

What do I need to bring to the appointment?
Copy of childhood immunization record

Childhood Lead Prevention
Children are at greater risk than adults for lead poisoning. Lead poisoning can cause permanent damage to a child’s brain, kidneys, and other organs. Exposure can lead to behavior problems, increased aggression and learning difficulties.
All children one to six years of age, particularly those less than 3years of age, are at risk for lead exposure due to frequent hand-to-mouth contact.
What services are available?
A blood lead test is available and the only way to know if your child has been exposed to lead. Screening children for lead poisoning is very important.

Uninsured children one to six years of age
Is there a cost for services?
Childhood Blood Lead Screening is FREE to eligible children who do not have Medicaid or a source of health insurance.
