APPLICANT MUST SUPPLY THE FOLLOWING
  • 3 sets of plans

    • Draw plans to scale
    • Describe the:
      • Layout and construction of the facility, 
      • Installation and design of equipment, 
      • Intended menu. 
  • The fee for this review is $50

Please label all equipment and areas and show the following items listed below
  1. Hot water heater (REQUIRED BY ALL ESTABLISHMENTS)

  2. Janitorial facility with hot & cold running water (can wash, mop sink, or other curbed facility (REQUIRED  BY ALL ESTABLISHMENTS)
  3. Utensil washing equipment, at least a 3-compartment sink with left and right drain boards OR a commercial dishmachine and a 2-compartment sink with left and right drain boards
  4. Handwash sinks in each  preparation area, including the bar area
  5. Toilet rooms and the number of fixtures (Refer to the Florida Building Code for minimum requirements)
  6. Water supply source (Municipal or Well?)
  7. Type of sewage system (Municipal Sewers or Septic Tank?) Contact the local Plumbing Department for Grease Trap requirements, Contact Environmental Engineering 467-4804 if connected to a septic system. Note: Prior approval from Environmental Engineering is required if connected to a septic system.  
  8. Seating Capacity, including any out door seating, if applicable
  9. Construction materials of the floors, walls, ceilings
  10. Intended menu and if catered, the DPBR licensed facility that will be doing the catering


All food and/or beverage service managers must be State-certified by passing a written examination

3 Plans for Review: One set of plans will be kept by the department for inspection purposes and the remaining sets will be returned for submission to the local building department. Follow all comments made on the plans to avoid delays in opening for business.
