Application for Birth Certificate Birth Certificates ordered through our regular mail order service take 10 to 14 business days to process and must include a self-addressed, stamped envelope. For an additional fee we also offer Expedite ($10) and Overnight ($21) Processing services.

IDENTIFICATION: Birth Certificates are not public record and may be issued to only certain individuals. Applicants for these certificates must present an official, picture identification card, such as their Driver’s License, Pass Port, or other State Identification card. All mail or facsimile orders must include a copy of the applicant’s Drivers License or State Identification Card.
BIRTH CERTIFICATES available for Florida Births only
Birth Certificates can be issued only to the registrant (the child named on the record) if of legal age (18), parent, guardian, or a legal representative of one of those persons or by court order. In the case of a deceased registrant, upon receipt of the Birth certificate of the decedent, birth certificate can be issued to the spouse, child, grandchild, sibling, if of legal age, or a legal representative of any of these persons, as well as to the parent.
Birth Certificates are available for persons born in any Florida county from 1930 to the present. For births older than 1930, Birth Certificates can be obtained from State Vital Statistics office in Jacksonville.
DO NOT SEND APPLICATION FOR NEWBORN UNTIL THE BABY IS ONE MONTH OF AGE
