Applying and Enrolling Information

One KidCare application to apply to all 4 programs

Applications process can take 4-6 weeks

All applications are sent to Tallahassee; county
coordinators do not have access to the state system

If parent/guardian receives a letter, contact the number provided on the
letter for further information

If an application is not completed correctly,
sections left blank, or not legible - it may not be processed

Attach a current W-2, or four weeks of pay check stubs to the application as
proof of income
